Coach / Volunteer: Waiver & Release of Liability *- Required for all coaches & on-field (game-day) volunteers in the TYFC

Coach: Cover Sheet *- Required for each coach

Org: Background Check Affadavit *- Required for each organization

Org: Concussion Affadavit *- Required for each organization

Org: Scholastic Eligibility Affadavit *- Required for each organization

Player: Championship Game Absentee Form *- Must be completed for any player not participating with team during a TYFC Championship game.

Player: Emergency Medical Information *- Reference for medical personnel in the event of an injury & parent / legal guardian not available.

Player: Home School Form *- This form must be completed for any children that are home schooled. A progress report must also be filed with an updated form between 9/15 - 10/15 of the current year.

Player: Medical Clearance Form *- Must be dated after January 1st of the current year & signed by a physician.

Player: Participant Tracking AA Division *- Player Participation, Tracking, & ID Card

Player: Resume Participation Medical Clearance *- Required to resume play after an injury & must be signed by a physician.

Player: TYFC Injury Report Form *- Required for all medically confirmed player injuries that result in missed game/practice time.

Player: Waiver & Release of Liability *- Required for all participants in the TYFC - THIS FORM MUST BE USED

Team: National MPR Form- Tracking sheet to confirm all players receive their minimum plays PRIOR to the end of the 3rd quarter in each game

Team: Tackle Football Roster- This roster must be used & all columns completed. Prior year versions & incomplete forms will be rejected and teams cannot practice or play until approved.

Team: Weekly Check-In Sheet- All rostered players must be validated prior to games by a member of the opposing coaching staff.

TYFC New Member Application *- Application form must be completed for consideration to join the TYFC
